You are required to complete a health screening prior to your arrival to the university campus. THIS IS MANDATORY. The health screening includes three parts: UHS Registration Form, Tuberculosis (TB) screening, and proof of required immunizations.  

Part One: University Health Services Registration

• Complete this registration form (PDF) and return to UHS by emailing it to UHSTracking@ucmail.uc.edu.

Part Two: TB Screening requires a blood test to rule out tuberculosis. This must be done before your arrival to UC. 

• Step 1: Complete this set of Tuberculosis Risk Assessment forms with your healthcare provider.
• Step 2: Return all completed and signed forms, along with required testing and results to UHSTracking@ucmail.uc.edu. Please add “ENCRYPT” to the subject line of your email. DO NOT upload to Bearcats Health app.
• Step 3: Your documents will be reviewed by University Health Services. If any additional steps are required, you will be notified by email. 

This screening must be done before your arrival to UC and within the 3 months prior to the start of classes. If you are unable to complete prior to your arrival, please contact UHSTracking@ucmail.uc.edu for next steps. 

Part Three: Proof of Required Immunizations

• Required immunizations are listed below.
• Take the UC Standardized Immunization form (PDF) to your healthcare provider to complete and sign. This must be done before your arrival to UC's campus.
• Use your UC username and password to access your Bearcats Health App account. 
• Prior to your arrival to UC, upload your completed immunization form (PDF) to Bearcats Health App, showing proof of all university-required immunizations. This must be completed before your arrival to UC. 

Your required immunizations include:

Hepatitis B: 3 Hepatitis B vaccine doses OR 2 Heplisav-B doses OR Hepatitis B surface antibody (HBSAB) titer*

MMR (Measles, Mumps, Rubella): 2 vaccine doses OR MMR titer*

Meningitis A (Meningococcal Quadrivalent): One dose if you are 16 to 21 years of age. Dose must be given on/after 16th birthday.

Tuberculosis: IGRA blood test (QuantiFERON or T-Spot)

Tdap (Tetanus, Diphtheria, Acellular Pertussis): One dose given within the past 10 years.

Varicella: 2 vaccine doses OR Varicella antibody (VZVIgG) titer*
