Student Vaccination & Health Screening Requirement

Overview and Instructions

 

Effective Fall Semester 2017 for UC Clifton 
Effective Fall Semester 2018 for All Campuses (UC Clifton, Blue Ash and Clermont)


To promote a healthy campus community, the University of Cincinnati (UC) has a student immunization and health screening requirement.  

You must complete this requirement if you are a student that meets one of the following criteria:

  • Incoming degree-seeking undergraduate, graduate and professional student.  (Note:  Requirements for health professions students already exist and are customized to the particular school.)  

    AND/OR
  • An incoming international or ELS student.  
The required vaccinations are:
  • Measles, Mumps, Rubella
  • Tetanus, Diphtheria, Pertussis
  • Varicella (Chickenpox)
  • Hepatitis B
  • Polio (Only required for those students who will be younger than eighteen (18) at the start of the semester)

The health screening consists of a brief tuberculosis questionnaire.  If you answer “YES” on the questionnaire, you will need to proceed to tuberculosis testing (e.g. blood test or skin test). 

To meet the immunization and health screening requirement, two (2) steps must be completed: 
Step 1:  Electronic Form Completion (through secure portal)
Step 2:  Licensed Medical Provider (MD, DO, PA, NP) Documentation

To access all forms (paper and electronic) and submit appropriate documentation, new students will be given access to a secure portal.
