UHS Return to Work or Accommodation Request Procedures

To help facilitate safe return to work with restrictions, return from a full days absence or more due to a work-related injury/illness or as directed by the UHS medical care provider for re-evaluation, employees are to call the UHS Holmes Location (Medical Campus) – Holmes Hospital - 4th Floor, at 513-584-4457, to schedule a return to work or accommodation request evaluation appointment.  As necessary, employees may also be seen at the UHS Holmes location without an appointment.

Employees required to visit UHS before returning to work must provide work release documentation from the treating medical provider at the time of the UHS visit. The documentation should specify the actual date the employee may return to work and any specific work restrictions or accommodations needed.  UHS is to specify the estimated duration for restricted work release as well as the date that the employee is to return to the UHS Holmes location for follow-up evaluation.

All employees with a work-related injury or illness that miss more than one full day of work or have work restrictions related to the injury/illness are to be evaluated by UHS before resuming work.  See UC Advisory 12.2 (Work-Related Injuries and Illnesses) - http://ehs.uc.edu/Advisories/Advisory_12_2.PDF. 

All employees released to work with documented work restrictions are to be evaluated by UHS at the Holmes location as well.

Upon completion of the UHS return to work or accommodation request evaluation, UHS provides a copy of the employee’s “Return to Work or Accommodation Request” form to the employee. The employee is directed to provide the UHS work release letter to the supervisor for review.  If restricted duty or workplace accommodations are needed, the supervisor completes the response section of the letter and faxes the response to UC Human Resources at 513-558-0676 or emails the form to LOA ADM.

If a supervisor/manager/director has a reasonable belief that an employee’s ability to perform the essential functions of his/her job is impacted by a medical condition (including surgery or hospitalization) or mental condition, the supervisor/manager/director is encouraged to contact the UC HR Sr. Leave Administrator (513-556-6381).

Reminder:
Employees required to report to UHS for return to work must provide documentation from their personal provider at the time of each visit. The release document should state release is without restrictions (full duty) or, if released with restrictions, cite specific restrictions and duration of same.
